PARTICIPATION WAIVER

Please read this waiver carefully and completely before signing. By submitting this form you acknowledge that you have read, understood, and agree to all terms.

1.Assumption of Risk

I understand and acknowledge that participation in fitness activities, athletic training, martial arts, and related programs at Get Right Performance involves inherent risks of injury, including but not limited to sprains, strains, fractures, and in rare cases serious injury or death. I voluntarily assume all such risks, known and unknown.

2.Release of Liability

In consideration of being permitted to participate in any and all activities offered by Get Right Performance, I hereby release, waive, discharge, and covenant not to sue Get Right Performance, its owners, coaches, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in any activity, or while on the premises.

3.Medical Conditions

I certify that I am in good physical condition and have no medical conditions that would prevent my safe participation in fitness activities. I agree to inform Get Right Performance staff of any medical conditions, injuries, or physical limitations prior to participation. I acknowledge that Get Right Performance staff are not medical professionals and that I should consult a physician before beginning any exercise program.

4.Minor Participants

If I am signing on behalf of a minor child, I represent that I am the parent or legal guardian of the minor and have the authority to execute this waiver on their behalf. I agree that this waiver applies to the minor participant and that I assume all risks on their behalf.

5.Photo & Media Release

I grant Get Right Performance permission to use photographs, video recordings, or other media in which I may appear for promotional, educational, or marketing purposes, including on social media and the facility website, without compensation.

6.Indemnification

I agree to indemnify and hold harmless Get Right Performance and its staff from any loss, liability, damage, or cost they may incur due to my participation in activities at the facility, whether caused by negligence or otherwise.

7.Governing Law

This waiver shall be governed by the laws of the State of North Carolina. If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.
